Most organizations think their AI governance program is more mature than it actually is. This guide introduces a practical AI governance maturity model — four stages from Developing to Advanced — explains what distinguishes each stage, and provides a self-assessment framework for identifying where your program has the most significant gaps.
Why maturity models matter for AI governance
Maturity models serve two functions in AI governance. First, they provide a realistic benchmark — most organizations overestimate their governance maturity, and maturity models create a structured basis for honest self-assessment. Second, they provide a roadmap — maturity frameworks show not just where you are, but what the next stage looks like and what it requires.
For regulatory purposes, maturity also matters. An organization that can demonstrate a Stage 3 or Stage 4 governance program — with documented processes, measurable controls, and continuous improvement cycles — is in a fundamentally different enforcement risk position than an organization at Stage 1 with ad hoc practices.
The four maturity stages
Developing — Ad hoc AI governance
AI governance is not yet formalized. Individual teams or employees make decisions about AI deployment without a defined process. There may be awareness that AI governance is needed, but no program is operational.
Indicators:
- No formal AI system inventory exists
- AI risk assessments are not conducted
- AI incidents are handled informally, case by case
- No governance policies have been adopted
- Executive leadership does not have visibility into AI use
Initial — Basic governance in place
An AI governance program has been started. An inventory exists, at least in draft form. Some risk assessments have been completed. Policies may exist on paper. But the program is not yet consistent, automated, or integrated into operational workflows.
Indicators:
- AI inventory exists but may be incomplete or in a spreadsheet
- Some systems have risk assessments; others do not
- Governance policies exist but may not be consistently followed
- Controls are tracked but not systematically monitored
- Incident management process is defined but not well-tested
Defined — Consistent, documented program
AI governance is formalized and consistently applied. Every AI system is registered. Every registered system has a completed risk assessment. Controls are implemented and tracked. Incident management is operational. Executive certification is in place. The program satisfies the core requirements of TRAIGA and comparable frameworks.
Indicators:
- Complete, current AI inventory maintained in a purpose-built system
- All in-scope systems have documented, auditable risk assessments
- Controls are assigned, tracked, and monitored for completion
- Incident management process is tested and operational
- Executive certification is documented and renewed on schedule
- Disclosure statements are generated and provided proactively
Advanced — Continuous improvement and proactive governance
AI governance is embedded in organizational culture and operational processes. The program is not just compliant — it actively improves AI outcomes. Governance data feeds back into procurement decisions, vendor management, and product development. The organization can respond to regulatory inquiries rapidly with audit-ready documentation.
Indicators:
- AI governance is integrated into procurement and vendor onboarding
- Governance maturity scores are tracked and improving over time
- AI incidents generate systematic improvements to risk assessments and controls
- Board-level reporting on AI governance is routine and substantive
- The organization can produce an audit-ready governance package on demand
Self-assessment framework
To assess your current maturity level, answer the following questions honestly:
- Can you produce a complete, current list of every AI system your organization uses in consequential decisions — right now, today?
- Does every system on that list have a documented risk assessment with an auditable classification rationale?
- Are the governance controls required for each system implemented and actively monitored — not just listed in a policy?
- If an AI incident occurred tonight, would your team know what to do, who to notify, and how to document it?
- Has your executive leadership reviewed and certified the AI governance program within the last twelve months?
- If a regulator called tomorrow requesting your AI governance documentation, could you produce it within 24 hours?
If the answer to any of these questions is “no” or “I'm not sure,” that is where to focus. Each “no” is a gap that an enforcement investigation can exploit.
Moving from Stage 2 to Stage 3
The most impactful maturity improvement most organizations can make is moving from Stage 2 (Initial) to Stage 3 (Defined). This transition requires:
- Completing risk assessments for every in-scope AI system — not just the ones that seem important
